Phase 1 — The Chronology
Walk the decision log chronologically. For each decision: confirm the record with the group. Ask who proposed it. Establish whether there was disagreement — the fact only, no interpretation yet. The numbers are less interesting than the process.
Phase 2 — The Design Comparison
"In the commission room this morning you described your service. You named your aircraft, your clinical team, your funding model. This afternoon your service met its first real test. Who were you then — and who were you now?"
On the design decisions
Which decision you made this morning created the most difficulty this afternoon? Was that foreseeable? What stopped you from seeing it?
On information management
When multiple incidents arrived simultaneously — what happened to the quality of communication in the team? Who held information? Who shared it? What did it cost either way?
On the hardest decision
Name the single decision today that you are least confident in. Not the worst outcome — the decision you are least sure about. What made it hard? What would you do differently and why?
On accountability
The commissioner will review your performance today. Three decisions are under scrutiny. Which three? Are you more or less confident about them now than when you made them?
On what this day produced
Across this morning and this afternoon — what has changed in how you understand what your team is capable of? Not what you learned about air ambulances. What you learned about yourselves in a room under pressure.
